Application for Advanced Level Certification

When a participant has completed the requirements of all courses of instruction for the Advanced Level of Catechist Certification (see the schema of the Diocesan Catechist Certification Program in this guide), it is his or her responsibility to notify the Parish Catechetical Leader or Catholic School Administrator that the requirements have been completed. The Parish Catechetical Leader/Catholic School Administrator will issue an application form to the participant to complete. When the form is completed and returned, the Parish Catechetical Leader/Catholic School Administrator will send the form to the Diocesan Office of Faith Formation: Catechist Certification. The Advanced Level Certificate will be issued upon verification that the requirements have been met.
