Group Life and Accidental Death and Dismemberment Insurance (ADD)
All Full-time and Part-time With Benefits employees are provided life insurance in an amount equal to an employee’s basic annual earnings up to a maximum of $100,000.
Accidental Death and Dismemberment Insurance (ADD) is provided as well, according to the terms outlined in the policy These programs are underwritten by Guardian. The amount of benefit paid is based upon the salary as reported in the Common Payroll System in IOI/Paylocity. Premiums for this coverage are paid by the Employer.
Coverage will become effective after one month of continuous active employment. Please keep your beneficiaries up to date.
